Findings
On or about July 15, 2021, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation in which W1 struck AV causing injury. An investigation determined that W1 has a history of aggressive behavior. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
