Findings
On or about July 19, 2021, Alleged Victim (AV) eloped from the secured memory care facility and was later found by Law Enforcement confused and disoriented sitting on a random porch of a home in the community. An investigation determined that a door at the facility was not latching correctly which resulted in AV eloping from the facility. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
