Findings
On or about July 20, 2021, the Alleged Perpetrator #2 (AP2) punched the Alleged Victim (AV) in the nose, causing a fracture to AV's nose. The AP2's actions are considered physical abuse and constitutes abuse. The facility's failure to protect AV from physical abuse is a violation of Oregon Administrative Rules.
