Findings
Alleged Victim (AV) relies on the facility for his/her care. The facility is to arrange medical transportation for AV. Between July 7th through 27th, 2021, AV had five (5) unattended medical appointment due to transportation issues. The facility failed to provide transportation for medical or social purposes, which is a violation of resident rights, is neglect of care and constitutes abuse.
