Findings
On or about July 23, 2021, the Alleged Victim (AV) left the locked facility after walking out the door, left open by construction workers doing remodeling on the second floor of the facility. AV was brought back to the facility unharmed, however, AV was placed at risk of serious harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
