Findings
On or about July 17, 2021, Unknown Alleged Perpetrator 2 (AP2) left Alleged Victim (AV) alone in his/her wheelchair. AV attempted to stand on his/her own, fell, and sustained injuries. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
