- The facility failed to intervene when the Alleged Victim‘s (AV) condition changed. On or about July 05, 2021, AV was found on the floor in h/h room after a fall. AV expressed pain at the time of the fall. Later that evening, AV had some red patches on back and tailbone and was unable to bear weight. The following day, AV had bruising and expressed pain. On or about July 07, 2021 AV was taken to the ER by family, and diagnosed with fractures to h/h pelvis. The facilities failure to assess AV and provide timely medical treatment is a violation or resident rights, is considered neglect of care and constitutes abuse.
