- Alleged Victim (AV) relies on the facility for his/her care. On or about July 25, 2021, AV was found at 9:11 am sitting on the bathroom floor with a small cut on his/her nose. AV was on the ground for 45 minutes or longer which cause AV to be upset. AV’s call pendant log shows a response time of 9 hours and 13 minutes that occurred on July 24, 2021, at 11:54 pm. The facility failed to answer the call light in a timely manner, which is a violation of resident rights, is neglect of care and constitutes abuse.
