Findings
On or about July 21, 2021, the Alleged Victim (AV) was given a shower by an opposite sex caregiver. AV's care plan states that he/she should be given showers by same sex caregivers only. AV was placed in a situation of feeling distressed and unreasonable discomfort. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
