Findings
The facility failed to protect Alleged Victim (AV) from a resident-to-resident altercation in which Witness #1 (W1) slapped AV. AV has a history of antagonizing behaviors towards W1 and W1 has a history of verbal outbursts. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
