Findings
On or about July 29, 2021, Alleged Perpetrator 2 (AP2) left Alleged Victim (AV) in the facility bus when AP2 went to help another resident get onto the bus. While AV was left unattended on the bus, AV stood up and fell out of the bus, about three (3) feet. AV suffered bruising and pain from the fall. AP2's action is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
