Findings
On or about August 5, 2021, Witness #1 (W1) ran into the Alleged Victim (AV) with his/her electric scooter, causing pain and a bruise to AV's hip. W1 had ran over AV's foot previously with his/her scooter. It had been recommended that W1 no longer use his/her scooter indoors, however, W1 continued to use the scooter. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
