Findings
The Alleged Victim (AV) was placed at the facility due to repeated falls. After admittance to the facility, AV continued to have falls and had at least 6 falls within a 2 month period. The facility failed to properly care plan and have meaningful interventions in place to reduce falls for AV, which placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
