Findings
On or about August 7, 2022, the facility failed to follow the Alleged Victim’s (AV) care plan that stated to assist AV with Geri-Sleeves in the morning and evening. The failure resulted in AV getting a skin tear to their right arm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
