Findings
On or about August 7, 2021, the Alleged Perpetrator #2 (AP2) spilled water on the floor of the whirlpool / spa room. AP2 was unable to clean up the water at the time due to working with another resident. AP2 did not lock the door or place "wet floor" signs to advise others of the wet floor. Later that day , the Alleged Victim (AV) entered the whirlpool / spa room and slipped, hitting his/her head and causing a break to his/her ankle, requiring surgery. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
