Findings
The Alleged Victim (AV) and Witness #1 (W1) were known to have altercations with each other and have had previous altercations. On or about August 9, 2021, AV and W1 had an unwitnessed altercation. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
