Findings
Alleged Victim (AV) has a known history of falls. During the month of June 2021, AV had sustained three (3) falls. On or about August 8, 2021, AV had an injury fall. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
