Findings
The facility failed to implement interventions and appropriately care plan related to AV's fall history. The failure resulted in AV experiencing approximately 10 falls in a 2 month period causing unreasonable discomfort and risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
