The facility failed to keep the Alleged Victim (AV) safe according to his/her known fall or leaning history. The failure resulted in AV being found lodged in his/her bed rail, causing injury and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
