Findings
On or about August 21, 2021, Alleged Victim (AV) fell from his/her lift chair. An investigation determined that AV suffered a similar fall on August 10, 2021. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
