Findings
The facility failed to provide a service for Alleged Victim (AV) which resulted in AV losing a significant amount of weight. Between February 2, 2021 and August 18, 2021, AV lost around 27lbs. The facility's failure to provide adequate oversight of AV's weight is a violation of resident rights, is considered neglect of care and constitutes abuse.
