Findings
On or about August 15, 2021, Witness 1 (W1) hit Alleged Victim (AV) in the eye while they were in a common area of the facility. W1 has known behaviors of verbal aggression and physical altercations with other residents. An investigation determined that facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
