Findings
Alleged Victim (AV) relies on facility staff to ensure resident's rights. On or about August 13, 2021, Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) failed to follow AV's care plan which included a walker while transferring AV and requires peri care after toileting. AP2 and AP3 transferred AV without the use of his/her walker, AV received an injury, and no peri care was administered after toileting. AP2 and AP3's actions are considered neglect of care and constitute abuse. The facility failed to ensure AV's care plan was followed which is a violation of Oregon Administrative Rules.
