Findings
On or about August 17, 2021, Alleged Victim (AV) was discovered by facility staff on the floor of his/her bedroom. An investigation determined that the facility failed to take necessary precautions to prevent AV from falling during the night which resulted in AV suffering a fall with a head injury. The facility's failure to properly care plan for AV's fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
