Findings
On or about August 19, 2021, the facility failed to ensure that Alleged Victim (AV) was receiving the basic care and supervision needed which resulted in AV falling, hitting his/her head, and obtaining a laceration that required sutures. An investigation determined that from June 2021 through August 16, 2021, AV had multiple falls and facility failed to implement fall interventions into AV's care plan. The facility's failure to properly care plan for AV's fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
