Findings
On or about August 2, 2021, the Alleged Victim (AV) received an as needed medication for a cream that staff were to apply two times per day for a yeast infection / itching rash in AV's peri area. The cream was applied one time in the month of August. AV was found to have redness and soreness in his/her peri area on August 26, 2021, causing unreasonable discomfort. The facility had been short staffed and unable to care for AV as needed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
