Findings
On or about August 24, 2021, the Alleged Victim (AV) was found outside, behind the facility. AV was uninjured, however, was placed at risk for harm. AV exited the facility through an unlocked gate, which had been unlocked by staff taking out the trash and wasn't locked when staff came back into the facility. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
