Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about August 20, 2021, AV sustained a fall in his/her room and was found between his/her bed and TV. AV had sustained two prior falls on July 29th and 30th, 2021 and the facility had no new interventions in place. The facility failed properly plan care for AV, which is a violation of resident rights, is neglect of care and constitute abuse.
