- Alleged Victim (AV) has a known history of falls. Between Jun 7th through July 3rd, 2021, AV sustained three (3) falls, AV has sustained a prior fracture due to his/her falls. On or about August 25, 2021, Av was found lying on the floor stomach down by the courtyard door with his/her wheelchair next to him/her. AV stated he/she was trying to stand up. AV had two skin tears to their left thumb/palm and left shin. On or about August 27, 2021, AV’s left wrist appeared swollen and painful after iced, AV was sent to the hospital and was returned to the community on August 31, 2021 with a fracture. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
