- Alleged Victim (AV) has a known history of having open areas to the skin in his/her legs and a history of skin infections in his/her legs. On or about August 6, 2021, staff assessed AV’s legs and documented they were weeping with three open areas. Between August 12th through 26th, 2021, AV refused dressing changes on six occasions. The facility did not provide documentation to show they had interventions in place to address AV’s refusals to allow staff to complete wound care and dressing changes. On or about August 28, 2021, staff smelled a foul odor in AV’s room and discovered the odor coming from AV’s legs. AV had the bandages wet and discolored, when staff unwrapped the bandages, maggots fell from them. AV was transported to the Emergency Room (ER) and diagnosed with an infection of the skin and was prescribed an antibiotic. The facility failed to care plan appropriately and implement interventions to mitigate AV’s refusal of service, which is a violation of resident rights, is neglect of care and constitutes abuse.
