Findings
On or about August 31, 2021, Alleged Victim (AV) eloped from the facility. He/she was found approximately an hour and half later, two (2) blocks away from the facility. AV sustained a scrape to the top of his/her foot. The facility failed to provide a safe environment for AV. The failures are a violation of resident rights, are considered neglect of care and constitute abuse.
