- Alleged Victim’s (AVs) service plan dated August 19, 2021, indicates staff are to redirect AV, as AV has history of wandering into other residents’ rooms, and taking other resident items. On or about August 31, 2021, throughout the shift AV had been walking into other resident rooms and taking other residents’ items. AV tried entering Witness 1 (W1) room and W1 pushed AV to the ground resulting in AV hitting h/h head causing a lump, swelling and unreasonable discomfort. Alert charting documentation indicates, staff to make sure AV does not walk into other resident rooms as this has been aggravating residents and can lead to physical altercations. On or about September 01, 2021, a physical resident to resident altercation occurred between W1 and AV because AV was entering other resident rooms. The facility failed to follow the care plan, and appropriately implement reasonable interventions to address AV’s increasing and ongoing wandering into other resident rooms, and prevent further altercations between W1 and AV, which is violation of resident rights, is considered neglect of care and constitutes abuse.
