Findings
The Alleged Victim (AV) suffered seven falls, with and without injury, from July 2021 to September 2021. On or about September 1, 2021, AV suffered a fall that resulted in being sent to the hospital and receiving seven staples in his/her head. The facility failed to properly care plan to ensure AV's safety, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
