Findings
The Alleged Victim (AV) was given an unwanted hug by Witness #3 (W3). AV complained to facility staff about W3, however, no interventions were put into place to diminish W3's behaviors. The facility failed to provide a safe environment, which is a violation of resident rights, considered neglect of care and constitutes abuse.
