Findings
Alleged Victim (AV) relies on the facility for his/her care. AV had five falls between June 16, 2021, and August 14, 2021, where some falls resulted in bruises and skin injuries. The facility failed to implement appropriate interventions to mitigate the risk of injury due to AV's increase in falls. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
