Findings
On or about August 17, 2021, AV had a fall and was sent to the hospital for treatment. Based on interviews and facility documentation, it was determined that the facility failed to assess, intervene, and monitor Alleged Victim (AV) when he/she experienced a change of condition which resulted in AV suffering pain and unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
