Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about September 1, 2021, Alleged Perpetrator 2 (AP2) lifted AV's legs up and down and did not stop when AV indicated it was causing pain. AP2s actions are considered physical abuse. The facility failed to protect a resident from physical abuse which violates Oregon Administrative Rules.
