Findings
The Alleged Victim (AV) was listed as independent on his/her care plan, however, AV is known to need assistance with mobility, toileting and general care. AV has had numerous falls, with and without injury due to not being assisted when necessary. The facility has been short staffed, leading to AV not getting the care he/she needs. The facility failed to properly care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.

RCFCP21-03405 $500.00 fine assessed
