Findings
The Alleged Victim (AV) is care planned to have a shower 3-4 times per week. The facility shower has been under construction for months, and in the past, AV has been taken to the Memory Care to have his/her shower. The Memory Care has been under executive order, staff have been unable to provide AV his/her shower. AV was to get bed baths, however, the facility has been short staffed and AV has not been getting bed baths regularly. AV has gone for more than a week without a shower, placing him/her in unreasonable discomfort. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
