Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about August 3, 2021, AV had a fall with injury. The facility failed to appropriately respond to AV's change in condition which resulted in several falls. This failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
