Findings
On or about August 5, 2021, Alleged Victim (AV) had a fall and temporarily lost consciousness from hitting his/her head. Alleged Perpetrator 2 (AP2) failed to provide appropriate health assessment, oversight, and monitoring when AV’s medical condition changed which resulted in AV experiencing an untreated decline in his/her condition and was sent to the hospital on or about, August 17, 2021. AP2’s action is considered neglect of care which constitutes abuse. The facility failed to provide appropriate health assessment, oversight, and monitoring when AV’s medical condition changed which resulted in AV experiencing an untreated decline in his/her condition. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
