- Alleged Victim (AV) has a history of falls, requires assistance with mobility and transfers. On or about, September 30, 2021, AV experienced an unwitnessed fall which resulted in knee pain. After the fall, AV pushed the call button several times and received no response from care staff. According to the activity log of AV's call button between September 1, 2021 and October 1, 2021 there were several calls where the reset times ranged from 52 minutes to 382 minutes. The facility failed to answer AV's call light timely which is a violation of resident rights, is considered neglect of care and constitutes abuse.
