- Alleged Victim (AV) relies on the facility for his/her care. AV has a history of agitation and aggression towards other residents and members of the care team. On or about September 09, 2021, AV had aggressive behavior towards a staff member, which lead to AV striking Witness #4 (W4) on the head causing W4 to fall to the ground. The facility failed to properly put interventions or safety plans in place for AVs aggressive behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
