Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about August 27, 2021, AV was found on the floor in his/her apartment and sustained a large bruise to his/her back. On or about September 10, 2021, AV was observed to have a bruise on his/her leg due to a second sustained fall on this day. A safety plan was not in place until September 14, 2021. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
