Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about September 7, 2021, AV was found by day shift staff with a soaked brief and bedding and AV had a skin tear in his/her groin area. AV’s brief was wet in the morning. NOC shift was supposed to toilet AV but didn’t. Alleged Perpetrator 2 (AP2) usually does the changing during the night shift. AV showed sighs that the skin tear caused pain. AP2 failed to provide service to AV, which is neglect of care and constitutes abuse. The facility failed to assure AV received service, which is a violation of Oregon Administrative Rules.
