- On or about September 19, 2021, the Alleged Victim (AV) and Witness #1 (W1) engaged in an altercation where AV was pushed by W1, resulting in a nasal bone fracture, bruising and facial lacerations. Earlier the same day, AV and W1 had been in a verbal altercation. Staff were to be checking AV and W1 at least hourly to avoid altercations. Before the second altercation, staff had not checked on W1 and AV for over an hour. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
