Findings
The facility failed to follow the Alleged Victim's (AV) service plan to preform safety checks every hour for falls. AV had approximately 6 falls prior to the injury fall. On or about September 3, 2021, AV was found on the floor with blood dripping down his/her face. AV was sent to the hospital and returned with stpales in his/her head. The facilities failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
