- On or about September 20, 2021, Alleged Perpetrator #2 (AP2) was seen on video popping a narcotic from a blister pack belonging to the Alleged Victim (AV), and then placing that pill into his/her mouth instead of giving the medication to AV. AV had earlier requested an as needed pain medication. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes financial exploitation. The facility failed to protect AV’s medications from theft. This failure is a violation of Oregon Administrative Rules.
