- Alleged Victim (AV) is care planned to received housekeeping service as needed. Photographs indicate there was a large black area on the wall in AV’s bathroom extending from the shower towards the toilet. Photographs also indicated what appeared to be items of debris on the floor on AV’s living room. There is no indication that staff were following AV’s service plan in regard to the cleanliness of AV’s apartment. The facility failed to follow the service plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
