- Alleged Victim (AV) relies on the facility for his/her care. AV has a history of thirteen (13) falls in less than two months. AV has sustained multiple bruises, skin tears, and has required two (2) trips to the emergency room. On or about September 06, 2021, AV experienced an un-witnessed fall resulting in skin tear to Av's left elbow. The facility failed to appropriately care plan and implement reasonable interventions to address Av's falls, which is violation of resident rights, is considered neglect of care and constitutes abuse.
